Prior to participation in Spring Arbor University Athletics, each student-athlete must register on the Athletic Training Department’s record-keeping system, Healthy Roster.

The pre-participation physical exam form should be printed and taken to a primary care physician’s office to be completed. Please use the SAU physical form. The first two pages are to be filled out by the student-athlete, and the third page is to be completed by a physician.

Physicals may be completed by any MD, DO, PA or NP. If you would like to get a physical completed near SAU during the summer, please contact the Athletic Training staff.

Visiting teams with special requests or accommodations are encouraged to contact the Athletic Training staff prior to arrival. If your team will be traveling without a Certified Athletic Trainer, please notify SAU Athletic Training in advance.
